Short term care
Instead of ongoing care you may only need support for a short period of time, for example while you recover after a hospital stay or to give your carer a short break from the caring role.
There are a few different short-term support options available, depending on your circumstances and your needs. The overall aim is to support you for a period of time with day-to-day tasks to help you (re)gain or maintain your independence.
Options include:
Respite care - Delivered in your own home, a community setting or in a residential care home, offering the opportunity for both you and your carer to take a break for a few hours, a day, a night or a few weeks.
Transition care - To help improve your independence and confidence at the end of a hospital stay, the Transition Care Program provides short term assistance either in your own home or a 'live in' setting such as an aged care home.
Short Term Restorative Care - If you have not been admitted to hospital but do need some help to regain your independence, the Short Term Restorative Care program is a flexible option that focuses on regaining wellbeing over a short period of time.
