At the assessment

For the assessment, a member of your local RAS or ACAT/S will visit you in your home or in hospital to assess your needs. 

You will be asked a series of questions to work out how much and what sort of help you require with daily and personal activities and to determine the best care option for your situation. 

The aged care assessor may also want to talk to your doctor, or other relevant health professionals, about your health and if there are any additional needs that should be considered.

Based on the information you provide they will create a support plan and also give information about the process of accessing supports, what costs are involved and where you can find providers to suit your needs.

During the assessment you can ask any questions you may have including about your care, the support plan or the assessment.

You can have a support person present, like a family member or friend, who can help you answer questions and to make sure you get all the information you need.
