A few relatively simple modifications can make your home a much easier place in which to live. Here are some of the areas where we can provide practical help: In the bathroom: full bathroom modifications, installing grab rails, shower curtain rods and hand held showers, removing shower screens. Around the house: installing grab rails, wedge ramps, lever taps and chair raisers, replacing washers, widening doorways, replacing lightbulbs, securing rugs and cords. Outside the house: installing handrails, safety lights and access ramps, small plumbing jobs. Any other repairs or modifications that will help ensure your safety.

INTENSIVE EIGHT WEEK PROGRAM STRC packages are funded by the Department of Health and are designed to reverse / slow the ‘functional decline’ of older people with difficulties in everyday tasks. The program aims to improve their wellbeing and quality of life. STRC is delivered at home by a multidisciplinary team of health professionals and allied health specialists to address any medical conditions. Sydney Community Services provides health services and community connections that support seniors and people with disabilities in Lane Cove, Hunters Hill, Ryde and surrounds to live independently and safely in their own homes. Services include in-home nursing and podiatry, delivered meals, shopping service and accompanied activities, domestic assistance - general house cleaning and linen service, gardening, home modifications and maintenance services, social calendar (including outings, lunches, movies, exercise, relaxation and games), medical transport, a Community Hub, and a range of Community Engagement and Participation programs for adults with disabilities. SCS is an approved My Aged Care and NDIS services provider.

Home Care ServiceOur Home Care Service offers home and community services to the Chinese, Vietnamese and South East Asian elderly community. Our aim is to promote wellness and reablement; helping elders to maintain their independence and living in their own homes.  Day to day living activities can become difficult to manage for most people as they grow older. We provide tailored person centred  services to enable you to continue  living in your own home.  Our services are not limited to   cleaning, meals, domestic assistance, personal care, home maintenance, transport, social support, shopping, access to allied health services, access to home modifications and maintenance service, but we engage in quality interpersonal and social interaction in a friendly manner.  Our Home Care package service comes in four levels from basic (Level 1) to high (Level 4). Delivered as ‘Consumer Directed Care’, we work with you to create the best package to help meet your needs and achieve your goals. This gives you more increased client choices and flexibility.  Our services are available across the region, including:  Personal care – showering, bathing, toileting, dressing. Support services – cleaning, laundry, gardening, transport, home maintenance. Clinical care – nursing and allied health. Nutritional support – help with meals. Continence support – products and equipment. Mobility – aids and specialised equipment. Leisure support – activities and interests as individuals or in groups Wellness and reablement program – health workshops and information sessions CHARGES We provide subsidised Home Care Package and non-subsidised (private) Home Care service.
