La Vita Care

La Vita Care is an in-home care provider specialising in hospital rehabilitation and neurological care and can assist people who have:

• Been in a road traffic accident (TAC).
• Had a stroke or have experienced a similar condition
• Are recovering at home from a recent hospital admission.
• Have been diagnosed with a neurological condition such as Huntington’s disease, motor neurone disease (MND), multiple sclerosis, Parkinson’s disease, epilepsy, Alzheimer’s, dementia or other related neurological conditions. 
• Have a condition resulting from a brain or spinal injury.
• Have been diagnosed with Autism Spectrum Disorder.

We help clients to live in their own homes and participate in their local communities with our support. Community access is a key focus for us. We do this by offering attendant care services, rehabilitation support, skills development services and community participation services.

We believe that community participation is essential to health and well being and improves people’s quality of life. We support people by acting as a companion so they can participate in community activities that they enjoy. 

Aged Care & In-Home Support

 At La Vita Care a sense of identity and quality of life are paramount to us, which is why we tailor our aged care services to ensure that they support the way our clients want to live.

We help people to live in their own homes and assist them to participate in their local communities. We not only offer In home care and personal care services, but can help with housekeeping, meal preparation, daily living tasks and access to community activities such as shopping and/or social activities.
