For most people living independently in your own home as you age is what you want. But sometimes you might need some help with daily tasks that you can no longer manage as well on your own.The Home Care Package Program provides packages of care and services to meet higher level needs and helps you live independently in your home. The Home Care Packages Program allows you to tailor a coordinated range of care and services to help you meet your care needs and achieve your goals.For example, you might need help with home maintenance, such as changing light bulbs. You might also need minor aids and equipment, like a walking frame, to help you get around your home safely and easily. Or you might need help with tasks like dressing or preparing meals. As you get older, you may also need some level of nursing care to stay in your own home.GWC Community Services also provides a variety of different Social Support and Centre Based Respite Groups for the frail aged, Greek speaking and Culturally and Linguistically Diverse (CALD) background, as well as people with dementia. The program is coordinated by highly trained staff and includes transport to and from the centres. All participants can expect to be treated with dignity and respect. Our aim is to promote wellness and reablement; helping elders to maintain their independence and living in their own homes.  Day to day living activities can become difficult to manage for most people as they grow older. We provide tailored person centred  services to enable you to continue  living in your own home.  Our services are not limited to   cleaning, meals, domestic assistance, personal care, home maintenance, transport, social support, shopping, access to allied health services, access to home modifications and maintenance service, but we engage in quality interpersonal and social interaction in a friendly manner.  Our Home Care package service comes in four levels from basic (Level 1) to high (Level 4). Delivered as ‘Consumer Directed Care’, we work with you to create the best package to help meet your needs and achieve your goals. This gives you more increased client choices and flexibility.  Our services are available across the region, including:  Personal care – showering, bathing, toileting, dressing. Support services – cleaning, laundry, gardening, transport, home maintenance. Clinical care – nursing and allied health. Nutritional support – help with meals. Continence support – products and equipment. Mobility – aids and specialised equipment. Leisure support – activities and interests as individuals or in groups Wellness and reablement program – health workshops and information sessions CHARGES We provide subsidised Home Care Package and non-subsidised (private) Home Care service.
