This includes people who are isolated, face communication or literacy barriers, struggle with decision-making, hesitant to engage with aged care, or risk a crisis situation. Resthaven’s Care Finder advisers are experienced social workers and aged care professionals who will listen, respect your individuality, and work with you to reach your goals.
The program specifically targets senior Australians who are homeless or at risk of homelessness or have multiple reasons for requiring intensive support to interact with My Aged Care, access aged care services and/or access other relevant supports in the community.
